The Personalised Care Team support individuals accessing NHS-funded care in a Care Home or in their own home, providing assessments and reviews for NHS Continuing Healthcare (CHC), Funded Nursing Care Contributions (FNC) and Children’s Continuing Care (CC).

  • Manchester Foundation Trust (MFT) covers our local hospitals; so if your concern relates to Trafford General, Wythenshawe, Manchester Royal Infirmary/ Eye or Dental Hospitals/ St Mary’s/ Royal Manchester Children’s Hospital - including CAMHS and Healthy Young Minds and some Trafford Community services, you can contact MFT PALS.

  • Trafford Community Services were provided by Pennine Care NHS Trust which merged with Manchester Foundation Trust (MFT)and became Trafford Local Care Organisation (TLCO) in October 2019 .
