Trafford Together Locality Plan for health and social care 2020-2025

The NHS and Trafford Council are working together on a joint plan for health and social care services.Organisations from Trafford Partnership and Trafford Local Care Alliance are also working with us.

The plan describes how, through the partnership working (known as Trafford Together) how we will aim to radically improve residents’ health and wellbeing and transform services over the next five years.

What have we been doing?

During 2019 the Trafford Together partners regularly met to share our ideas and plans. In November 2019, we produced our first draft of our Trafford Together Locality Plan for health and social care 2020-2025. It is hoped that we will refresh this plan every year building on our engagement.

Local services can provide better and more joined-up care for people when different organisations work together in this way. For our colleagues, improved collaboration can help to make it easier to work with other organisations. By working alongside and drawing on the expertise those listed above we can help people to live healthier lives for longer, and to stay out of hospital when they do not need to be there.

You can help us

Whilst the plan builds on lots of engagement work we’ve done over the last few years, we want to make sure it reflects everything that is important to local people.

As partners across the borough, we will be coming out to Trafford communities throughout 2020 to work with providers of public services, patients and residents so we can understand your views and make the plan come to life for you.

We will update this website and Trafford Partnership website on a regular basis with opportunities to get involved.
