Accessing your healthcare information

NHS Trafford CCG respects the rights members of the public have in obtaining information that is held by our organisation.

Our publications page and Freedom of Information (FOI) page provide more detail on what information is readily available and what maybe reasonably withheld. These two pages also explain why the NHS collects information about you: how it may be used, how we keep your personal information secure and confidential, and your general rights of access to information held about you.

What kind of information can I request?

If you have not been able to find the information you require on our website, then in principle you can request any kind of information the organisation holds. To help understand the different types of information available see blow:

Personal Information

To make a request for personal information you must be the person who the information is about or have written authority from that person if you are acting on their behalf, examples of information available, could be your personal details, name, address etc, copies or access to your health record.

To make a request for personal information please fill in our Subject Access Request form.

Making your request in writing

If you wish to make you request in writing then please write to:

  • First Name(s)
  • Family Name
  • Address
  • Description of the Information you would like a copy of
  • In what format you would like to receive information (e.g. paper copies, electronically, view files on site)

Accessing Patient Medical information

To request a copy or to view your records then your request must be made in writing to the manager of the organisation who holds the information you are after e.g. doctor’s surgery, hospital, dentist, optician, etc.

Caldicott Guardian

The Caldicott Guardian has responsibility for ensuring that the principles of confidentiality are applied throughout the business of the CCG, and for considering any applications to share information. The Caldicott Guardian acts as the conscience of the organisation and champions patients’ information rights.
